Large cream coloured paper certificate with a red paper seal; green and detailed border surrounds the content of the certificate; at the top and center of the certificate, and inside the border, is a crest of the Ministry of Health for the Province of Ontario; below the crest outlines that Mary Benedict is bestowed the certification of a registered nurse, who has the power to bestow this certification, their signatures, and the date of the certification; on the left and center side of the certificate and inside the border is the certificate number; and below the certificate number is a large red paper stamp from the Minister of Health.
