Single-breasted box style nurse’s ‘walking out’ cream wool jacket with a notched collar; three 2.1 cm white plastic button closure; full cream polyester lining; gold twisted piping over 1.0 cm green grosgrain ribbon along all edges including pockets; ladies size 8; machine-embroidered Ontario Hospital of Whitby crest on right waist pocket and initials HBC on left breast pocket; sleeves from shoulder to cuff at finger press line 60.0 cm; bust is 98.0 cm; centre back at neck to hem is 71.2 cm.

Hazel Bentley Pritchard is a graduate of the Ontario Hospital School of Nursing, Class of 1968.
Ontario Shores Centre for Mental Health Services is located in Whitby, Ontario. The hospital was founded in 1919. The OSC still operates today as a mental health facility (2013). The hospital was the site on treed farmland by the shores of Lake Ontario provided patients fresh air, sunshine, space to walk and an opportunity to heal. The hospital provides a range of specialized assessment and treatment services for those living with complex and serious mental illnesses.

Description
Wheelchair safety belt, which consists of two main flat nylon straps, both loose at one end, and connected to each other at their other ends to form a V-shape; a semicircular closed metal buckle is sewn to each strap approximately 56 cm from their loose ends; there is a white rectangular label sewn to one strap; a short piece of strap connects the two larger straps below these buckles; below this portion, the straps slant towards each other, and are sewn together at their ends; a folded piece of strap is sewn to this connection point; three rows of stitching provide it with three loops.

Condition Remarks
The edges of the midpoints of the straps were yellowing; the bottom, folded extension had a yellow and white deposit on its top surface; there were three brown deposits (small) on the top surface of the large straps near their loose ends; there were no tears or corrosion on metal parts.
