Dr Ralph and Mrs Olga Crawford donated their extensive Canadian dental collection to the DCF to create the museum in 1997; further donations were received while Dr Crawford was Curator Emeritus at the Dental Canada Museum until its closure in 2008; this item was donated by Dr. W. Kozakewich from Calgary, Alberta; this collage was created by a patient of his, Paige Smith, in appreciation of his removing her wisdom tooth; the collage is made up of found objects she collects from garage sales and flea markets, which she then arranges in composition to express an emotion; the old receipt numbered 48 represents wisdom tooth number 48, and the stamped date, 01 June 1998, represents the date it was extracted
