Dental tooth-form set in brown wooden case; rectangular case (a) with two yellow metal hinges and locking clasp closures; white rectangular paper with manufacturer's information, instructions and photographs of each tooth-form, printed with black and red ink, affixed inside lid; white paper label covers bottom of case, numbers each tooth-form; cut-out circles and spaces for brown metal tooth-forms and tools; cylindrical swager (b) with one flat end, one round end; one hollow cylindrical ring (c); wrench with cylindrical handle attached to bolt with octagonal end (d); 150 tooth-forms (e) with various shaped models of each tooth mounted on cylindrical bases; white sheet of paper (f) listing number of tooth-forms with corresponding shell crown numbers; white envelope (g) with manufacturer's address printed with blue ink at top left corner

Dr Ralph and Mrs Olga Crawford donated their extensive Canadian dental collection to the DCF to create the museum in 1997; further donations were received while Dr Crawford was Curator Emeritus at the Dental Canada Museum until its closure in 2008; this item was donated by Dr. John Robertson from Summerside, Prince Edward Island; the 1915 catalogue entry for this product states that a seamless gold shell could be shaped into a crown ready to trim in five minutes
