Certificate printed with black ink on cream or light brown paper; the name of the group with the authority to certify the record is found at the top of the certificate in large capitalized letters and between the name of the group is the group's crest, which features an eagle next to a shield with the American flag on it; below the name and crest of the group are the words, written in cursive, describing the nature of the certification and what it entails, whom the certificate is directed to, and who is providing this certification; at the end of the paragraph and at the bottom of the certificate are five signatures and the certificate number; at the bottom left of the certificate is the seal of the Illinois State Board of Nurse Examines; surrounding the content is a well-designed border; and while the back of the certificate is originally blank, at the top left corner, written in pen, is further information pertaining to the people in the photo.

Originally belonged to the donor's great-aunt, Harriet Peck, who is a graduate from the Kingston General Hospital School of Nursing, Class of 1900.

The edges of the certificate are heavily torn, ripped, curled and bent, but the content of the certificate and seal is still intact; misisng pieces along edges and corners; a few stains on the front, which are showing on the back, and one of the words written in pen on the back of the certificate is missing due to tearing

Research Facts
The Act of June 30th, 1913 referred to on the certificate created a board of five registered nurses, known as the Illinois State Board of Nurse Examiners. They were appointed by the state governor with the consent of the state. The members of the board were paid ten dollars a day plus travel expenses when working. One member was to be appointed secretary and receive a salary of 1400 dollars a year.
