Rectangular commemorative bookmark with a forked end printed in blue ink on reverse side of decorative vinyl fabric; front design contains the Ontario trillium logo printed in the top left corner in a box, and below that the year 1886 printed on a diagonal going across the bookmark from the left up towards the top right enclosed in a ribbon design; below that is a printed circle with the words "Ontario Hospital Kingston" printed inside it along the edges of the circle in capital letters; middle of the circle contains Ontario's crest with a cross at the top and the three maple leaves near its bottom; below the circle is printed the year 1971 in the same style as 1886; bottom right corner of the bookmark, in one side of the fork, are printed the letters "KPH" in a ribbon design.

Used by donor’s great aunt, Connie Cox (20 Aug 1939- 5 June 2010), while working at the Kingston Psychiatric Hospital for over 35 years. Ontario Hospital Kingston Nursing School, Class of 1962.
Ontario Hospital Kingston gained this name in 1920; before it was Rockwood Hospital. In 1965 the name changed again to Kingston Psychiatric Hospital (what the "KPH" stands for on the bookmark). Since 2001, it has become part of the Providence Care Centre.
