Ballenger's swivel septum knife, which consists of a hollow, four-sided handle that has a closed end formed into a rounded, blunt point, and flat sides with horizontal gripping grooves on the outer top and bottom edges; the handle tapers to join with a solid, cylindrical shaft that splits and flares out to form an elongated U-shaped tip that has a small, swivelling, curved blade at the top; the inside edges of the tip are flat and the outer edges are rounded.

The top of the handle has the number "4" stamped into the metal just before it joins with the shaft, as well as the F.A. Hardy & Company logo of a human eye with the letter "C".
