This apostle’s spoon would be part of a spoon collection, and the size of the spoon bowl is the right size for medication. It was not originally used as a medicine spoon, just as a stand in. Many spoon collections have a variety of bowl sizes, this just happened to be the right size for the owner and happened to be an apostle’s spoon.
An apostle spoon is a spoon (usually silver or silver-plated, but sometimes of other metals, such as pewter) with an image of an apostle or other saint as the termination of the handle, each bearing his distinctive emblem; a full set is twelve spoons.
The letters EPNS stand for "electro-plated nickel silver"
