Made at the Hospital for the Ruptured and Crippled, later renamed the Hospital for Special Surgery (in 1963) in New York City. Dr. John Hazlett required it for back pain. He wore it until the problem diminished and he could no longer fit into it. Dr. Hazlett and his wife (donor; nursing graduate from Toronto Western hospital) lived in the Bronx while he was a Fellow at the Hospital for Special Surgery.
