Dental tooth-form set in brown wooden case; rectangular case (a) with two yellow metal hinges; white rectangular paper with manufacturer's information, instructions and photographs of each tooth-form, printed with black and red ink, affixed inside lid; white paper label covers bottom of case, numbers each tooth-form; cut-out circles and spaces for brown metal tooth-forms and tools; cylindrical swager (b) with one flat end, one round end; two hollow cylindrical rings (c, d); wrench with rounded handle (e); silver metal dentimetre tool (f) for measuring teeth, with octagonal handle and wire loop at top; wrench with cylindrical handle attached to bolt with octagonal end (g); 150 tooth-forms (h) with various shaped models of each tooth mounted on cylindrical bases

Dr Ralph and Mrs Olga Crawford donated their extensive Canadian dental collection to the DCF to create the museum in 1997; further donations were received while Dr Crawford was Curator Emeritus at the Dental Canada Museum until its closure in 2008; this item was donated by Dr. Glenn Walker from London, Ontario; the 1915 catalogue entry for this product states that a seamless gold shell could be shaped into a crown ready to trim in five minutes
