"[FORM D] // ARTICLES OF AGREEMENT // Between a Licentiate of Dental Surgery and a Student of Dentistry // (Form approved by the Board of Directors of the Royal College of Dental Surgeons of Ontario, March 8th, 1878) // Articles of Agreement made in triplicate, the Third // day of August in the year of our Lord, one thousand, eight hundred // and eighty six // Between Martin E. Snider of the // City of Toronto in the County of York and // Province of Ontario, Licentiate of Dental Surgery of the Royal College of Dental Surgeons of // Ontario, of the first part, and Henry P. Martin // of the same place, Student of Dentistry, of the Second part. // Witnesseth that the said party of the second part hath placed, and by these presents doth // bind himself, to the said party of the first part to serve him as a Student in Dentistry for the full // term of two and one-half years, from the date hereof; excepting for the period of eight months //during the said term of two and one-half years, when he is required to attend Lectures in the // School of Dentistry at Toronto. //And the said party of the second part doth hereby covenant with the said party of the first // part that he will during the whole of the said term of term of two and one-half years faithfully and // diligently serve the said party of the first part as a Student of Dentistry, and obey all his reason // able commands, excepting during such portion of the said two and one-half years as he shall be // absent attending Lectures in the City of Toronto. // And the said party of the first part doth hereby covenant with the said part of the second // part that he will be according to the best of his skills and ability, teach and instruct, or cause to be properly taught and instructed by some other duly qualified and competent person or persons, the // said party of the second part in the knowledge and practice of all subjects and branches which are, // or may be, from time to time required by the Statutes relating to the Profession of Dentistry, and // by the Rules, Regulations and By-laws of the Board of Directors and Examiners of the Royal // College of Dental Surgeons of Ontario, and that such teaching and instruction shall be given to // the said party of the second part at the office of the said party of the first part in the said // City of Toronto in the County of York. // aforesaid (except during so much of the said term of two and one-half years as may be spent by // the said party of the second part in attending Lectures on Medicine or Dentistry) and that he will // not permit the said party of the second part to practice Dentistry during the said term unless under // his own immediate personal instructions and supervision. // In Witness Wherof the said parties have hereunto set their hands and seals the day // and year first above written. // NOTE These Articles to be signed and sealed in triplicate, one copy for each of the parties, and the third one to be trans - // mitted to the Secretary of the Board immediately after signing, to be filed in his office."
