Clear glass bottle that once contained Formolid Antiseptic Solution; bottle has flat sides with a seam running diagonally around the bottle; rounded shoulders lead upwards to a thin, circular neck which curves outward to form a round lip; there are three white labels on the bottle; a rectangular one stuck to the front and the back and a small rectangular one on the proper left hand side; the label on the front is printed on in blue ink with a red decorative border around the edge of the label also printed in ink and contains product composition and company information; the label on the back is also printed on in blue ink with the same red decorative border and contains directions for use; the label on the side of the bottle is printed on only in blue ink and indicates that a new label style was adopted in 1909
