An (LP) 33 1/3 R.P.M. vinyl record with paper sleeve; cream and white square cardstock cover, (a); series name, description and names of those involved in the creation of the vinyl are printed in yellow and white ink on cover; the title is in yellow ink; the manufacturer symbol and name is in black ink at the bottom-right corner on the front cover; back cover contains the title in black ink in the upper left corner and a short description of the contents of the record, as well as four monochrome black and white photographs of the doctors involved on the right side and their names and credentials below them. On the left is a blue stamp for the Nova Scotia Hospital. Inside the covers is a black vinyl disc (b) with a burgundy label in the middle on both sides, which includes the series, title, and manufacturing information in silver ink. There is also a paper sleeve in which the record sits, (c).
